Contact the REAR COMMODORE or Fill in your information on the FORMS listed below!

Before you start get all you club info and put it on the paper form. 

Print the Paper Form Work Sheet

•  First select your club from the dropdown list.

•  Enter your pass code (pass code is your District Letter and Membership number)

• To find your pass code go here -  Member Clubs

•  Fill in all available information from your work sheet and answer questions. 

•  THEN SUBMIT!!!  You may always go back to the form to check your work or make         changes.
